Meet your physician

Dr. Doug Briggs has practiced relational medicine for nearly 40 years.

A UNC-trained family physician, board-certified since 1991, Dr. Briggs trained and practiced in Dallas, spent 23 years caring for families in southwest China, and in 2018 came home to Raleigh as founding chief medical officer of NeighborHealth Center. In 2025 he received the Harris Memorial Award, given each year to one North Carolina physician.

There is a universal need for physicians who have a longstanding commitment to treating patients as individuals within the context of their families and communities.Dr. Doug Briggs

Where his approach took shape

Care that starts with knowing the person in front of you.

For two decades in rural Yunnan, where specialists and hospitals were often hours away, Dr. Briggs ran a walk-in clinic, made house calls and trained village doctors. It was demanding medicine, and he kept his U.S. board certification current throughout.

The lesson he brought home is the one this practice is named for: good medicine happens face to face, with time to listen to the whole family.

    For employers

Insurance is for catastrophes. Your team needs a doctor.

Small businesses in Raleigh can offer Face-to-Face memberships as a benefit: a predictable monthly cost, fewer sick days and quick answers for employees and their kids.

    Questions

    Frequently asked.

    What is direct primary care?

Direct primary care (DPC) is a membership. You pay a flat monthly fee directly to the practice, and that covers your visits, access to your doctor by phone and text, and a long list of in-office services. There are no copays and no insurance billing.

Taking insurance out of primary care means a much smaller patient panel, roughly 600 patients instead of 2,500, so Dr. Briggs has time for each one.

Do I still need health insurance?

You don't need insurance to become a patient. Anyone can enroll, insured or not.

But if you have health insurance today, we don't recommend canceling it to join, and direct primary care isn't designed to replace catastrophic coverage. Membership covers your everyday primary care; insurance is still what protects you for hospital stays, specialists, surgery and emergencies. A high-deductible or catastrophic plan pairs well with DPC.

    Do you see children?

Yes. Face-to-Face is a family medicine practice for patients age 6 and up, so kids, parents and grandparents can all see the same doctor. Children enroll with a parent or another adult in their household. Each child is $60 a month, and the fourth child and beyond are free. Any household of two or more members, including a parent with one child, gets 10% off its total monthly fee.

A parent or guardian signs for children under 18. Teens 16 and older can come to visits on their own, and as they grow, we make room for some time one-on-one with the doctor.

Do you accept patients with existing medical problems?

Absolutely. Diabetes, high blood pressure, asthma, heart disease, anxiety and depression, or several conditions at once: caring for chronic conditions is a big part of family medicine. With longer visits and a direct line to Dr. Briggs, he can work closely with you to manage them.

    How do I get started?

Enrollment is open now. You can schedule a free meet and greet to get to know Dr. Briggs first, or enroll online in a few minutes. Membership is limited on purpose; a small panel is the whole point.

    How does billing work?

Memberships are billed monthly by card or bank draft, with a one-time $99 enrollment fee per household. There's no long-term contract: you can cancel with 30 days' notice. Labs are billed at cost, with no markup, and in-office procedures have posted prices.

    Can I use my HSA?

Often, yes. Starting January 1, 2026, the One Big Beautiful Bill Act and IRS Notice 2026-5 changed the rules for direct primary care in two ways. First, being a DPC member no longer disqualifies you from contributing to an HSA alongside your high-deductible plan. Second, you can pay the membership fee with HSA funds.

Both apply only when total DPC fees are $150 a month or less for one person, or $300 a month or less for a membership that covers more than one person. Those limits are adjusted for inflation after 2026. Every individual Face-to-Face membership is under $150. A larger family, such as two adults and three or more children, can come to more than $300 a month; if yours would, talk with us before you enroll.

Membership fees don't count toward your health plan's deductible or out-of-pocket maximum.

We accept HSA cards. Making sure HSA funds are used for qualified expenses is the account holder's responsibility, so check with your plan administrator or tax advisor if you're unsure. This isn't tax advice.

I'm on Medicare or Medicaid. Can I join?

Yes, you're welcome. Dr. Briggs has opted out of Medicare and the practice doesn't bill Medicaid, so you'll pay the membership yourself and sign a short private agreement. Your Medicare or Medicaid coverage still applies to hospitals, specialists and everything outside our office.

    Isn't this just concierge medicine?

No. Concierge practices usually charge a large annual fee and still bill your insurance. DPC doesn't bill insurance at all, which keeps memberships well below typical concierge fees while giving you more of your doctor's time.

    What isn't included?

Imaging, specialist visits, hospital and emergency care, and prescriptions aren't part of the membership, though we help you find the lowest cash prices for them. Vaccines and pulmonary function testing are referred out.

Know your doctor. Let him know you.
